  1. Each insurer makes it's own decision on whether to operate in a particular state and what specific products to offer - usually driven by state laws.

    "Affordable life insurance" is relative. The state might have regulations that stop life insurance that doesn't pay the full face value right away.

  3. Because the company chooses to exclude them for business reasons. If a company's policies do not comply with a state's regulations, that company may simply choose not to do business in that state.
